Transition Service (Thurrock Council) supports all young people between 14 and 25.

The service ensures that young people with special educational needs have a professional (if needed) to support, guide and inform them and their parents/carers about options and opportunities for the future.

The service supports young people to make informed choices and decisions with support from those who know them best and offers support to enable them to live independently as much as possible and to access education, training, employment opportunities and leisure and recreation activities.
