This service is provided by the University College London Hospitals to help bridge the gap during transition from paediatric to adult services. Patients are referred to the service alongside their usual consultant support.

They offer specialist multi-disciplinary reviews tailored to your needs and provide a long term point of contact for any concerns or queries.

The service will help co-ordinate your care with other community services, specialist centres and healthcare professionals.
